SERVICES

West Seattle Foot & Ankle Clinic sees patients for foot and ankle pain related to use, injury, deformity, and/or acute infection. We see patients of all ages for:

INSURANCE
|
Bring your insurance card(s) with you to each appointment. For a list of insurance we are contracted with, visit the Insurance page.

|
REFERRALS
|
If your insurance company requires a referral to a specialist, you must first contact your primary care physician and have a referral faxed to our clinic.
|
|
CO-PAYS
|
Insurance regulations dictate that any co-payments required by your policy must be paid at the time of service.
|
|
CANCELLATIONS
|
If you need to cancel an appointment, please provide 24 hours notice. Late cancellations and no-shows are subject to a $50 account reinstatement fee.

|
MASKS
|
In accordance with Washington State guidelines, masks will be optional starting April 3, 2023. You may request that your provider wear a mask during your appointment at our clinic.

PRIVACY |
We value your privacy and want you to be informed of how we may use and disclose your protected health information. For specifics, please read our Notice of Privacy Practices prior to your appointment. Data submitted through the Registration portal will be transmitted securely and in compliance with applicable laws and regulation.

SERVICE ANIMALS

Animals are prohibited in the clinic expect for those defined as "service animals" under the Americans with Disabilities Act.
