Billing Statements

First Text & Email
Patients will receive a text from (855) 734 -3792 and/or an email when there is a balance due.

Second Text & Email
After a few days, a friendly text and email reminder will be sent to patients who have not made a payment towards their balance. 

Paper Statement
If a patient has not paid online, a paper statement will be sent in the mail.  

Pay Online With Your Statement

When you have your statement in hand, simply click the link below to pay your bill. You will be prompted to enter the Online BillPay Code from your statement and the Amount Due.

After you pay, you have the option of creating an account to save your payment data securely in our payment system.

Pay Online as a Guest

If you don't have your statement on hand, you can still pay online. Simply click the button below and you will be prompted to enter your demographic data.
FIRST AND LAST NAME
Enter the full legal first and last name of the person financially responsible (guarantor) for the bill in the respective First Name and Last Name fields. Avoid any extra spaces at the beginning or end of the name.

DATE OF BIRTH
Enter the person financially responsible's (guarantor’s) complete date of birth in the format MM/DD/YYYY.

STREET ADDRESS
Enter the person financially responsible’s (guarantor’s) full street address. You can enter the address in any format you prefer, such as ‘123 Main Street’ or ‘123 Main St.’
